Kelleher is the anglicised form of the Gaelic Ó Céileachair, a name that means "companion-dear" — a compound of the Old Irish words for companion and beloved. It belongs to the Munster province and is among the most distinctively Cork surnames in Ireland.

The original Uí Céileachair were a sept belonging to the Dál Cais tribal grouping — the same dynasty that produced Brian Boru, High King of Ireland. They held territory in the barony of Muskerry in County Cork, the heartland of the MacCarthy kingdom. The Kellehers served as sub-lords and stewards within the MacCarthy Mór's extended territories.

After the Norman conquest of Munster in the 12th century, the Kellehers retreated further into the Cork hinterland. The surname survived the Cromwellian plantation relatively intact — partly because the family were not large landowners and partly because west Cork's terrain made it difficult to subdue. Kellehers participated in the Flight of the Wild Geese (1691), with documented members serving in Irish brigades in France.

By the 18th and 19th centuries, Kelleher was one of Cork's most common surnames. The Great Famine (1845–1852) drove substantial Kelleher emigration to the United States, particularly to Boston, New York, and Chicago.
