The principal Gaelic origin is Ó Leannáin, a sept from County Galway in Connacht. The name Leannán may derive from the Old Irish word for "cloak" or "sweetheart" (leannán, meaning beloved one). The Ó Leannáins were lords of the barony of Loughrea in south Galway before being displaced by the Anglo-Normans.

A second origin is found in Ulster: Mac Giolla Fhinnéin, meaning "son of the devotee of Saint Fionnán." Saint Fionnán was a 6th-century monk associated with Lough Erne in County Fermanagh. This sept's name was anglicised as both Leonard and sometimes as McGillfinnan before being absorbed into the Leonard anglicisation.

Norman families named Léonard also arrived in Ireland during the 12th and 13th centuries, settling primarily in Leinster and Connacht. These families blended with the Gaelic Leonards over time, making definitive separation of origins difficult without specific family research.

The Leonard family produced one of Ireland's most celebrated and controversial 20th-century playwrights: Hugh Leonard (pseudonym of John Joseph Byrne, 1926–2009), author of Da — one of the most acclaimed Irish plays of the century, which ran on Broadway and won multiple Tony Awards.
